CONSENT FOR USE OF PERSONAL INFORMATION: I hereby authorize theKingsley Heath (Pty) Ltd's and / or Secotrade 61 (Pty) Ltd's ("Company") dulyauthorized verification agents ("Agents") to access my Personal Informationand conduct background screening checks including, but not limited to, credit,qualifications, employment references, criminal record, fraud prevention, IDverification, polygraph check and drivers’ licence. I consent to requests forconsumer credit information to be released for the below prescribed purposesonly: For employment in a position of trust and honesty and entails thehandling of cash or finances; Fraud prevention or detection. I understand thatverification requests form part of the background screening process and: Thatrequests for credit information from Credit Bureaus will only be conductedunder the regulations defined as per the NCA; Data obtained from the FSBserve only for the purpose to determine the fitness and propriety as envisagedin the FAIS Act. I acknowledge that any Personal Information supplied to theCompany is provided voluntarily and that the Company may not be able tocomply with its obligations if the correct Personal Information is not supplied tothe Company. I understand that privacy is important to the Responsible Partiesand the Responsible Parties will use reasonable efforts in order to ensure thatany Personal Information in their possession or processed on their behalf iskept confidential, stored in a secure manner and processed in terms of SouthAfrican law and for the purposes I have authorised. I warrant that allinformation, including Personal Information, supplied to the Company isaccurate and current and agree to correct and update such information whennecessary. By submitting any Personal Information to the Company in any formI acknowledge that such conduct constitutes a reasonable unconditional,specific and voluntary consent to the processing of such Personal Informationin the following manner by the Company and/or verification informationsuppliers: Personal Information may be shared by the Company with its agentsfor the purposes detailed above. Personal Information may be stored for areasonable period by the Company, and its agents. I take note that if theResponsible Party has utilised the Personal Information contrary to the Privacyand Data Protection Conditions, I may first resolve any concerns with thatResponsible Party. If I am not satisfied with such process, I have the right tolodge a complaint with the Information Regulator. A copy of PersonalInformation kept by the Responsible Parties will be furnished to me uponrequest in terms of the provisions of POPI or the NCA and I understand that Imay dispute any information in the record provided. I unconditionally agree toindemnify the Responsible Parties, and Verification Information Suppliers,acting in good faith in taking reasonable steps to process my personalinformation lawfully, against any liability that may result from the processing ofmy personal information. This includes unintentional disclosures of suchpersonal information to, or access by unauthorized persons, and/or anyreliance which may inadvertently. I have the right to object to the use of the Personal Information at any time and such objection is to be lodged with the

[email protected]. In this event, the informationshall be destroyed and I acknowledge that I shall no longer be considered for
any position with the Company.
